At LISEGA, INC., we encourage all employees to choose healthier lifestyles and participate in education, screenings, and regular visits with medical professionals to take care of ourselves and our wallets.
In order to avoid a 50% increase in the medical premiums for this year, employees and their spouses can participate in the annual survey and biometrics offered onsite for your convenience and paid by LISEGA, INC.
Participation in the onsite biometrics is voluntary and available to all eligible employees and spouses. If you choose to participate, you will be asked to complete the Personal Health Survey, which asks a series of questions about your health‐related activities and behaviors and whether you have or had certain medical conditions (e.g., cancer, diabetes, or heart disease). You are not required to complete the Personal Health Survey, but if you do, you will receive the benefits discussed above. The survey responses and biometric results will develop a Personal Health Report to help you understand your current health potential risks and provide evidence based information to reduce those risks as well as guide you in a healthier lifestyle. We encourage you to share your results or concerns with your own medical provider.
All health information is kept strictly confidential. LISEGA, Inc.’s Human Resources and Administration will NOT have access to your personal health information. The program is managed solely by Covenant Health and administered through a third‐party company, Applied Health Analytics, which partners with health systems across the country to enhance the health of their communities. Applied Health Analytics is bound by HIPAA laws and will not share your personal information.
